By transitioning to an electronic medical record solution, we’re improving the operational, clinical and financial success of our company’s home health and hospice agencies.


Care transition

In hospitals around the country, our dedicated care transition coordinators serve as a vital link between physician, hospital, home health agency and patient – facilitating the transition from acute to post-acute care. From medication management and post-discharge education to follow-up appointment coordination, our care transition coordinators help improve patient outcomes and reduce avoidable hospitalizations.


Disease management

Our approach to patient care starts with education. We teach our patients the tools they need they need to self-manage and identify red flags that may cause their condition or symptoms to worsen – promoting early intervention and greater independence.


Call Us First

To reduce unnecessary ER visits and prevent readmission, we teach patients to call us first when problems or symptoms occur. Our on-call nurses are armed with triage protocols to respond appropriately to patient needs. We also offer the Philips Lifeline medical alert service for qualified home health patients as part of the plan of care. When a patient pushes the Lifeline personal help button, he or she will be contacted right away by a local on-call nurse.


Telehealth

Telehealth is a proven tool to reduce avoidable hospitalization and ER visits. By continuously monitoring and assessing our patients’ vital signs, we can improve clinical outcomes for individuals with heart failure, COPD, diabetes, hypertension and other chronic conditions.


Beyond Balance

Our comprehensive Beyond Balance program focuses on reducing the risk of injury and falls for at-risk patients or those who have fallen in the past.  We take a holistic, individualized approach to balance and mobility, providing in-depth treatment strategies designed to help promote functional mobility.


Behavioral health

For seniors living with diminished mental health, proper care can be the difference between golden years and stolen years. Our compassionate medical social workers and licensed nurses use a multidisciplinary approach to build comprehensive behavioral health plans that help patients and families live the healthiest life possible.


Patient call back

Our commitment to the patients in our care doesn’t end with discharge. A member of our team will make two calls to patients after home health services have ended to check on their recovery and progress. This service is one more way we’re helping keep our patients out of the hospital and in the comfort of home.
