From medication management and post-discharge education to follow-up appointment coordination, our care transition coordinators help improve patient outcomes and reduce avoidable hospitalizations.

With the Philips Lifeline medical alert service, provided as part of the plan of care to certain qualified patients, a local on-call nurse is available any time of the day or night to help manage our patients’ pressing health issues. When a patient pushes the Lifeline personal help button, he or she will be contacted right away.
Telehealth is a proven tool to reduce avoidable hospitalization and ER visits. By continuously monitoring and assessing our patients’ vital signs, we can improve clinical outcomes for individuals with heart failure, COPD, diabetes, hypertension and other chronic conditions.

A member of our team will make two calls to patients after home health services have ended to check on their recovery and progress.
