Admission Process

Patients can be referred to our LTAC hospitals by a variety of sources including physicians, social workers, case managers, nursing homes, family, caregivers or the patient themselves. Qualified staff will perform a prescreening evaluation. On-site assessments may be performed. We accept Medicare, Medicaid and most private insurances. Pre-admission team conferences can be arranged to discuss patient and family goals. If necessary, transportation will be arranged by the hospital.
