their medical conditions.
CHRISTUS VNA Community Care
At CHRISTUS VNA Community Care, we are more than an organization of home and community based professionals – we are proud members of communities across San Antonio. Quality service is our mission. And to us, service means getting to know our patients on a personal level.
We know the people we serve, understand their needs, and tailor healthcare to ensure the best journey possible.
We’re passionately committed to making healthcare better every day by leading the industry in clinical excellence and service quality.
We empower our clinicians and staff to embrace the communities they serve, because the best care happens in the context of strong relationships.
Each patient and family we are privileged to serve is unique. Our ongoing mission is to design a specific plan to help those in our care achieve their best possible health – in the comfort of home.
Under the direct supervision of the referring physician, our highly skilled home care professionals deliver quality, compassionate care. We also include families and caregivers in the care process, offering valuable information, guidance and – most of all – support.
Pediatric care – A child’s illness, injury, or special needs can put stress on the entire family, and it can be incredibly challenging for families to navigate the process of caring for their child. Our clinical team can provide high quality personalized care to your patients and their families in the place where they are most comfortable – home. Home and Community Based Services are covered through Medicaid, long-term care, and some medical insurances. We accept most insurance. Please contact a location near you and we can verify your benefits and what your carrier will cover. Out-of-pocket services range in cost depending on the care you or your loved one requires.
Let us be an extension of your care. Our person-centered approach demands working with you to create the best journey possible for our shared patients.

Offer patients support on their journey toward independence.
Transitions are challenging — especially when they involve your own health, or the health of someone you care for. Discover the resources and support you need right in your own community.

This is the way healthcare is meant to be. You can be empowered to provide the kind of service you can be proud of. Our emphasis on person-centered care affords the time and the opportunity to make a real difference in people’s lives — every day.
