Our innovative Care Model has changed the delivery of Home Health Care in the State of Maryland, focusing on the needs of the patient as our primary concern. This approach has allowed us to be uniquely successful in transitioning patients from acute care and sub-acute care, to home.
We provide specialized training to all our clinical staff on chronic care best practices, evidence based treatment guidelines for diabetes, COPD, CHF, and depression, as well as Motivational Interviewing principles and Health Coaching skills, that encourage and improve self-behavior management. Our emphasis is on patient-centered care and forming partnerships with our patients to better engage them in their health and self-care.
Our recruiting philosophy follows suit, as we implement a stringent three step hiring and screening process, as well as a continual education and re-evaluation process of our existing staff. This commitment to staff training ensures that the highest levels of quality and a culture of excellence are attained and maintained.

Generally, Medicare and/or Medicaid will cover the cost of home health services for those patients who meet either program’s eligibility requirements. Private insurance companies, managed care organizations, and workers’ compensation plans may also pay for home health services.
