Home-Based ServicesHome Care - With health care costs continuing to rise, cost-effective solutions such as home care are becoming more and more popular. Not only does it allow families to keep medical costs down, it also helps the healing process by allowing you to recover in the comfort of your own home. Hospice - Our hospice program provides comprehensive comfort care for patients with life-limiting diseases. Our multidisciplinary team addresses both the care of the patient as well as the emotional needs of the family. Telehealth Services - We offer Telehealth Services to our patients utilizing technology as an extension of your care. These services include our Personal Emergency Response System (PERS), a one-touch button device worn in case of emergencies or need for immediate assistance, and Telemonitoring, a program that takes one or more of a patient’s vital signs every day using select monitoring devices. Other Telehealth Services include patient care advice, prevention methods and health education. Through these services, your loved one or you are able to take a more proactive approach to recovery. Private Duty - Our private duty program is an affordable and personalized care option offering a wide range of services to patients at home or in a medical facility. The type of services utilized depends on each patient’s specific needs – for acute or chronically ill patients, we have a medical professional dedicated to their care. We work with patients and their families to customize services based on their needs. |
