We offer a wide array of disease management programs that deliver clinical results in areas such as balance and fall prevention, behavioral health, and chronic disease management. And in many of our partner locations, specially trained care transition coordinators seamlessly transition patients from the acute to the post-acute setting. In each of our service lines, we employ objective criteria to measure the quality of care we provide.

Our results speak for themselves. In 2017, more than two-thirds of our home health locations across the nation – a total of 217 – were named among the nation’s best as part of the 2017 HomeCare Elite®. This independent ranking recognizes the top-performing and most successful home care providers in the United States.

In April 2018, our quality and patient satisfaction ratings – according to the latest Centers for Medicare and Medicaid Services (CMS) Five-Star Quality Rating System results – again outperformed national averages and industry peers. LHC Group achieved a quality score of 4.68/5 stars in the April report – an improvement over its 4.55 score in the January report. This compares favorably with a national average that has been within a range of 3.25 to 3.28 for the last five quarters. LHC Group’s April patient satisfaction score was 4.32, compared to the industry average of 3.70.

Finally, we are a national leader in Joint Commission accreditation. Whereas just 15 percent of home health agencies nationwide have achieved Joint Commission accreditation, 100 percent of our home health and hospice locations have earned accreditation or are completing the accreditation process.​
